Meningeal Worm

Meningeal worm is a deer parasite that alpacas, llamas, sheep and goats can pick up from slugs and snails, causing sudden and serious neurological disease.

Meningeal worm, often called brain worm or m-worm, is a roundworm named Parelaphostrongylus tenuis. Its natural home is the white-tailed deer, where adult worms live in the membranes around the brain (the meninges) and usually cause the deer no trouble at all. In areas where the parasite is established, roughly 80% of white-tailed deer are thought to carry it. Deer pass worm larvae in their droppings, slugs and snails pick those larvae up, and the larvae mature inside the slug or snail into the stage that can infect a new animal. The cycle closes when a grazing deer accidentally swallows an infected slug or snail along with a mouthful of grass.

Problems start when the animal that swallows that slug is not a deer. Alpacas, llamas, sheep and goats are what vets call aberrant or dead-end hosts: the worm cannot complete its life cycle in them, so instead of settling politely in the meninges it burrows through the tissue of the spinal cord and sometimes the brain. The physical damage and the inflammation that follows are what make the animal ill. Signs usually show up around one to two months after the slug was eaten, and they typically begin at the back end and work forward. Things to watch for include:

  • Weakness, wobbliness or dragging in the hind legs
  • Difficulty standing up, or sitting down in odd positions
  • Head tilt, an arched neck, circling or stumbling
  • Gradual weight loss, or a sudden change in how the animal moves

Two points matter for owners. First, an affected alpaca or goat cannot pass the parasite to its pasture mates, and the worm is not a risk to people. Second, there is no reliable blood or fecal test in these species, so a vet works from the clinical picture, the season and the local deer population, and often begins treatment on suspicion rather than waiting for proof. Treatment combines a dewormer with anti-inflammatory medication and good nursing care. Animals caught early can recover, sometimes with lasting neurological deficits, while advanced cases are much harder to turn around. That is why any sudden change in coordination in a camelid or small ruminant deserves a same-day call rather than a wait-and-see week.

Prevention is far more effective than treatment, and it works by interrupting the life cycle. In regions where the parasite occurs, common approaches include preventive macrocyclic lactone injections (such as ivermectin or doramectin) during the months when slugs and snails are active, plus fencing deer away from pasture and grain, keeping grass and damp debris where gastropods shelter under control, and not feeding at ground level. Exact products, doses and intervals vary by species, country and drug licensing, so build the plan with your own vet.

Geography shapes the risk. Meningeal worm follows the white-tailed deer and is a concern in eastern and central North America, with the recognised range shifting westward and northward. It is not an issue for pet owners in Europe. If you have moved animals or are new to keeping camelids or small ruminants, asking a local vet whether the parasite is present in your area is a sensible first step.
